AHCCCS Medicaid:
UnitedHealthcare Community Plan • Arizona Complete Health • Health Choice Arizona

Self-pay is also available for patients who prefer not to use insurance or whose insurance is not currently accepted.

Self-Pay Rates:
Initial Psychiatric Evaluation: $300
Follow-Up Appointments: $150

Payment is due at the time of service.
